Forms   All request forms must be signed by the District Arts-In-Education Representatives in order to be processed. When using PDF files be sure you may fill out the information on the form and print it out. Your information will not save when you close the form. Please be sure you have printed before closing the form. When the forms are printed, be sure to send it to your District Arts in Education Representative for authorization. They will then forward it to our office. Only at this point will a contract be generated.
